What is the founding member discount?
The first 100 members to join Rooted Wellness & Care in Arizona receive permanently locked-in founding pricing: Rooted Essential at $69/month (regular $89), Rooted Complex Care at $99/month (regular $129), and Weight Loss Membership at $79/month (regular $99). Your founding rate never increases as long as your membership stays active. If you cancel and rejoin later, you will be charged the current rate at that time. This offer is for Arizona founding members only.
Who actually responds to my messages?
Your provider — directly. There is no office staff, no triage nurse, no medical assistant relay. When you send a message to Rooted, your board-certified NP reads it and responds personally. That's the whole point of this model.
Do I need to be in Arizona for visits?
Yes. Arizona law requires that you be physically located within the state of Arizona at the time of any video or phone visit. This applies even if you are an Arizona resident temporarily traveling. Secure messaging between visits does not have the same requirement, but all clinical decisions and prescriptions are based on Arizona licensure.
What are your hours?
We offer video visits, phone calls, and secure messaging Monday through Friday. We also check messages Saturday and Sunday mornings. Time-sensitive messages are responded to same business day, or next business day if received overnight. Routine messages—refills, questions, follow-ups—are responded to within 48 hours. We are not an emergency service—for emergencies call 911 or go to your nearest ER.
How does care actually work day-to-day?
We use a mix of video visits, phone calls, and secure messaging—whatever fits the situation. Need to talk something through? Schedule a video or phone visit. Have a question, need a refill, or want to share a symptom update? Send a message. Time-sensitive messages are responded to same business day, or next business day if received overnight. Routine messages within 48 hours.
Is this health insurance?
No. This is a direct primary care (DPC) membership—a method of delivering primary care without insurance billing. We recommend pairing with a low-cost catastrophic or health-sharing plan for hospital coverage.
Can I use my HSA or FSA?
Yes — as of January 1, 2026, DPC membership fees are qualified medical expenses under federal law (the One Big Beautiful Bill Act), meaning you can pay your Rooted membership with pre-tax HSA dollars. All three of our membership tiers are under the $150/month individual limit. This effectively makes your membership 20–30% cheaper depending on your tax bracket. Acute care visits and weight loss program fees are also HSA-eligible. Note: FSA eligibility for DPC memberships varies by plan administrator — check with yours. We provide itemized receipts for all payments.
What conditions do you manage?
Hypertension, type 2 diabetes, hypothyroidism, high cholesterol, asthma, GERD, anxiety, depression, ADHD, and more. Complex cases receive coordinated referrals while we stay your primary contact.
Do you prescribe controlled substances?
No. Rooted Wellness & Care does not prescribe controlled substances of any kind. This includes opioids, benzodiazepines, stimulants (such as Adderall), sleep medications, and any other DEA Schedule II–V medications. If you currently take a controlled substance managed by another provider, we are happy to coordinate care alongside them but cannot prescribe or manage those medications ourselves.
How much do GLP-1 medications cost?
Medication costs are separate from program fees. Brand-name Wegovy/Zepbound start at $149–$299/month with manufacturer savings cards. We help navigate the best current options given evolving FDA compounding rules.
What platform do you use for visits?

Can you order labs and imaging?
Yes. We order labs through Sonora Quest and LabCorp at discounted cash-pay rates—often 80–90% less than insurance-billed rates. Imaging orders written for cash-pay centers statewide.
What if I need to see a specialist?
We provide referrals with detailed clinical notes and maintain communication with your specialist to coordinate care—something most primary care offices don’t have time to do.
Can I cancel my membership?
Yes—any time, for any reason. No cancellation fees, no penalties. We’d rather earn your continued membership through excellent, compassionate care.
Do you accept Medicare?
Not at this time. We are unable to enroll patients who are enrolled in Medicare Part A, Part B, or a Medicare Advantage plan (Part C). Federal law requires providers to formally opt out of Medicare before seeing Medicare beneficiaries on a cash-pay basis, and we have not yet completed that process. We hope to offer this option in the future.
Do you offer menopause care and hormone therapy?
Yes — Rooted Menopause Care is a dedicated service for perimenopause and menopause management. We prescribe FDA-approved estrogen and progesterone therapies when clinically appropriate, manage symptoms, coordinate labs, and provide ongoing monitoring. We do not currently prescribe testosterone. Menopause care is available as a standalone membership ($79/month founding, $89/month standard), as a $29/month add-on for Essential members, or included at no extra charge in Complex Care memberships.
What does the $179 initial menopause evaluation include?
The initial menopause evaluation is a comprehensive 45-minute telehealth visit covering your full symptom history, menopause staging, hormone therapy candidacy, cardiovascular risk assessment, review of any outside labs or records, and a personalized treatment plan. The $179 fee applies toward your first month of membership if you enroll — so your first month is effectively covered by the evaluation fee.
What hormone therapies do you prescribe?
We prescribe FDA-approved estrogen and progesterone therapies including oral, transdermal patch, topical gel or cream, and vaginal formulations when clinically appropriate. Medications are prescribed through your pharmacy of choice and billed separately. We do not currently prescribe testosterone or compounded hormone preparations. We do not prescribe pellets.
When would I be referred to a specialist for menopause concerns?
We manage the majority of perimenopause and menopause needs via telehealth. We refer to in-person specialists for: new or unexplained vaginal bleeding, new breast symptoms, significant mood or psychiatric concerns, complex thyroid or adrenal conditions, and new cardiovascular symptoms such as chest pain, shortness of breath, or leg swelling. If a referral is needed we'll provide a full clinical summary to your specialist.
Do you offer annual wellness exams and physicals?
Yes. Your annual wellness visit is done via telehealth using home-measured vitals plus a review of your health history and any lab work. Certain hands-on exam components aren't part of a telehealth visit — if something warrants an in-person exam, we'll refer you to a trusted local provider. Note: school physicals, sports physicals, DOT physicals, and any visit requiring a signed physical form by law must be done in person and will be referred to a local clinic.
Can you give vaccines?
We don't administer vaccines directly, but we'll send a prescription or order to your pharmacy of choice — CVS, Walgreens, Walmart, and most major Arizona pharmacies are authorized to administer routine vaccines. This is usually a quick walk-in visit for you at no extra charge from us.
Can you do Pap smears or prostate exams?
We can order FDA-approved self-collect HPV testing through our lab partner — done at a local draw site, no in-person exam required — which is the current standard for many patients' primary cervical cancer screening. For PSA prostate screening, we order bloodwork directly. If you have an abnormal result, symptoms, or prefer an in-person exam, we'll refer you to a trusted Arizona urologist or OB/GYN with a full clinical summary.
